The final distribution of benefits to eligible Class members took place on July 17, 2008. Class members will have 180 days from the issue date on the check to cash or negotiate the check.
If you need to request that the check be reissued for any reason, you must do so in writing and include any appropriate documentation to support your request. To request your check be reissued, include your full name, address, claim number (if available), daytime telephone number with the best time of day to reach you, and a statement explaining the reason for your letter.
